I blame lettuce. It almost never adds anything to the experience, and when combined with ketchup, mayo, etc it creates a smooth surface to make the other fillings shoot out when you bite into it.

i'd say burgers should be wider, not higher
but then they'd have a lower filling to bread ration and i don't like that.
Also the idea is that each bite should have all the ingredients, so the limiting factor really just is "how many different things can we fit into an open mouth's height?" Which makes it harder. I am for using thinner, higher-quality patties as a solution, but people wouldn't like that as much as a solution.

Yes, that's what happens when they make the buns too hard. Almost always a problem with bagels as well.
The trick is to hold the buns horizontal while biting, burgers never fall over ever since.
