It's the lighting. There are two light sources off screen with some sort of gobo, but they're at slightly different angles so it makes the shadows overlap.

I was extremely tempted to say this was aigen, and wrote a whole paragraph explaining that, but a tineye search suggests this image existed circa 2015, which as the world in general knows is before aigen tech was this capable. my primary source of belief that this was aigen was the jarring text, which proves to be a result of translation; the cartoon was apparently originally in russian, and the original text looks much more natural. see tineye results.
