Literally, though iirc they just used a club.
- More realistically, a good deal of alcohol.

I have this romantic notion they’d have a couple gulps of amber coloured spirits before the barber/surgeon would carve out the rotten parts.
- But, what if, perchance, like me, it is the brain that is rotten?
  
  Trepanning will do. Might take an extra gulp first.
Didn't they use large doses of nicotine?
- I think it was more like a large amount of whiskey, a cloth to bite on and 3 to 5 people to hold you down whilst they sawed off your leg as quickly as possible.
  
  If you were lucky, you'd get laudanum, which is a mixture of opium and alcohol.
- Opium. That and a solid strip of leather to bite down on.
