Shocking idea it may perhaps be because
- McDonalds "food" tastes like shit
- McDonalds "food" costs a ton of money
- There are burger spots (especially local ones) that can match or beat McDonalds pricing for significantly better burgers
- People can not afford to spend obscene amounts of money of food
